{"id":1192,"date":"2022-02-24T14:00:50","date_gmt":"2022-02-24T13:00:50","guid":{"rendered":"https:\/\/www.celiacscatalunya.org\/blog\/?p=1192"},"modified":"2022-02-24T14:00:50","modified_gmt":"2022-02-24T13:00:50","slug":"el-dilema-de-la-introduccio-de-la-civada-en-el-tractament-de-la-malaltia-celiaca","status":"publish","type":"post","link":"https:\/\/www.celiacscatalunya.org\/blog\/el-dilema-de-la-introduccio-de-la-civada-en-el-tractament-de-la-malaltia-celiaca\/","title":{"rendered":"El dilema de la introducci\u00f3 de la civada en el tractament de la malaltia cel\u00edaca"},"content":{"rendered":"\n<p>En aquest article recollim el treball de final de M\u00e0ster de Nutrici\u00f3 i Salut de Crist\u00f3bal P\u00e9rez Sixto, responsable del departament T\u00e8cnic de l\u2019Associaci\u00f3 Cel\u00edacs de Catalunya, que tracta sobre la introducci\u00f3 de la civada en el tractament de la Malaltia Cel\u00edaca (MC).<\/p>\n<p><span style=\"font-size: 20px;\"><strong>Antecedents<\/strong><\/span><\/p>\n<p>La civada s&#8217;ha presentat en els \u00faltims anys com un cereal molt nutritiu dins de la dieta occidental. Un aliment ric en carbohidrats d&#8217;absorci\u00f3 lenta, fibres i prote\u00efnes vegetals. A m\u00e9s, \u00e9s un cereal interessant que podria incrementar el valor nutricional de la Dieta Sense Gluten (DSG). La problem\u00e0tica radica en el fet <strong>que en l&#8217;\u00faltima d\u00e8cada s&#8217;ha posat en dubte el seu \u00fas com a aliment sense gluten en la dieta de les persones amb Malaltia Cel\u00edaca (MC).<\/strong> Aquesta problem\u00e0tica sembla que es podria plantejar a partir de dues hip\u00f2tesis:<\/p>\n<ol>\n<li>Una possible reacci\u00f3 autoimmune de les persones amb EC al consum del cereal, o;<\/li>\n<li>La contaminaci\u00f3 creuada de la civada amb altres cereals amb gluten en plantes de producci\u00f3 i envasament dels cereals.<\/li>\n<\/ol>\n<p>L&#8217;<strong>objectiu<\/strong> principal de la revisi\u00f3 va ser revelar si el cereal pot incloure&#8217;s o no en la DSG. Un altre objectiu secundari va ser explicar el mecanisme d&#8217;acci\u00f3 de les prolamines t\u00f2xiques dels cereals i demostrar si \u00e9s aplicable a la civada.<\/p>\n<blockquote>\n<p><em><span style=\"font-size: 20px;\">En l&#8217;\u00faltima d\u00e8cada s&#8217;ha posat en dubte l&#8217;\u00fas de la civada com a aliment sense gluten en la dieta de les persones amb malaltia cel\u00edaca\u00a0<\/span><\/em><\/p>\n<\/blockquote>\n<p><span style=\"font-size: 20px;\"><strong>Mecanisme d&#8217;acci\u00f3 de les prolamines<\/strong><\/span><\/p>\n<p>El gluten dels cereals es compon de dues parts, una part no t\u00f2xica (denominada glutenines) i una part t\u00f2xica en alguns cereals (les prolamines). En les persones amb MC, les prolamines no digerides aconsegueixen la paret intestinal i desencadenen una s\u00e8rie de reaccions que acaben degradant les microvellositats de la paret intestinal.<\/p>\n<p><span style=\"font-size: 20px;\"><strong>Diferents cereals, diferents prolamines<\/strong><\/span><\/p>\n<p>En funci\u00f3 dels cereals el percentatge de prolamina i la tipologia \u00e9s diferent. En la taula 1 veiem quina prolamina i en quina concentraci\u00f3 es troben les prolamines en els cereals.<\/p>\n<p><em>Taula 1. En aquesta mena de civada la literatura cient\u00edfica descriu que no desencadena la resposta autoimmune en pacients amb MC.<\/li>\n<li>Amb major contingut d\u2019avenines. En aquest cas alguns estudis han demostrat que pot desencadenar la resposta autoimmune en pacients amb MC.<\/li>\n<\/ol>\n<p>Es descriu, a m\u00e9s, que el mecanisme d&#8217;acci\u00f3 d&#8217;aquestes avenines t\u00f2xiques seria similar al de la gliadina del blat, aix\u00ed i tot, \u00e9s necessari realitzar m\u00e9s estudis per a corroborar aquestes primeres hip\u00f2tesis.<\/p>\n<p><strong>La metodologia de l&#8217;estudi es va basar<\/strong> a realitzar una revisi\u00f3 sistem\u00e0tica en la base de dades PUBMED. La cerca inicial ens va proporcionar 80 refer\u00e8ncies i finalment van ser 13 els articles inclosos en la revisi\u00f3. Aquests articles van ser assajos cl\u00ednics, casos i controls i estudis observacionals.<\/p>\n<p>L\u2019<strong>extracci\u00f3 de dades<\/strong> d&#8217;aquests estudis va determinar el seg\u00fcent: :<\/p>\n<ul>\n<li>Els pa\u00efsos en els quals s&#8217;han dut a terme major nombre d&#8217;estudis sobre el tema han estat Finl\u00e0ndia, Noruega, el Canad\u00e0, el Regne Unit, Su\u00e8cia i It\u00e0lia.<\/li>\n<li>Els models d&#8217;estudi van ser:\n<ul>\n<li>Estudi de casos i controls (1)<\/li>\n<li>Observacionals i transversals (5)<\/li>\n<li>Assaig cl\u00ednic unic\u00e8ntric (4)<\/li>\n<li>Assaig cl\u00ednic multic\u00e8ntric (3)<\/li>\n<\/ul>\n<\/li>\n<li>El nombre de subjectes totals inclosos en els 13 estudis va ser de 2176 pacients amb EC, dels quals 1092 van ser adults (&gt;18 anys) i 1084 nens (&lt;18 anys).<\/li>\n<li>La metodologia de la introducci\u00f3 de la civada en la dieta dels subjectes d&#8217;estudi va ser variable en tots els estudis. Es va determinar una ingesta d&#8217;entre 20 i 100 grams de civada al dia (en forma de flocs, pa o galetes).<\/li>\n<li>Un dels punts m\u00e9s importants i que marca la feblesa d&#8217;alguns estudis \u00e9s que no tenen en compte les varietats de civada utilitzades i no les analitzen pr\u00e8viament per a saber el contingut de gluten.<\/li>\n<\/ul>\n<p>Els<strong> resultats <\/strong>dels estudis i la <strong>interpretaci\u00f3 de les dades<\/strong> es van recollir en la seg\u00fcent taula:<\/p>\n<p><em>Taula 2. Aquestes reaccions adverses s&#8217;han descrit a trav\u00e9s de tres marcadors estudiats que van donar positiu:<\/p>\n<ul>\n<li>An\u00e0lisi i recompte de limf\u00f2cits intraepitelials en bi\u00f2psies duodenals.<\/li>\n<li>An\u00e0lisi de l&#8217;expressi\u00f3 d&#8217;ARN missatger en bi\u00f2psies duodenals.<\/li>\n<li>An\u00e0lisi d&#8217;\u00e0cids grassos de cadena curta (metab\u00f2lits de la microbiota) en femta.<\/li>\n<\/ul>\n<p>Un altre punt important a tractar \u00e9s que cap dels estudis ha mostrat variacions destacables al consum de civada en proves serol\u00f2giques. Encara que, d&#8217;altra banda, els estudis que detecten marcadors positius al consum de civada, no realitzen proves serol\u00f2giques en la detecci\u00f3 de marcadors.<\/p>\n<p><span style=\"font-size: 20px;\"><strong>Llavors, podem incloure la civada en nostra DSG? Conclusions dels estudis:<\/strong><\/span><\/p>\n<p><em>Il\u00b7lustraci\u00f3 1. Els estudis conclouen que la civada \u00e9s segura? D&#8217;aquesta manera, els pacients amb MC han de ser monitorats de prop per a evidenciar si existeix o no un problema de toler\u00e0ncia del cereal.<\/p>\n<blockquote>\n<p><span style=\"font-size: 20px;\"><em>No podem indicar amb seguretat que la civada pugui ser inclosa en la dieta de totes les persones amb Malaltia Cel\u00edaca (MC). Els pacients amb MC han de ser monitorats de prop per a evidenciar si existeix o no un problema de toler\u00e0ncia del cereal.<\/em><\/span><\/p>\n<\/blockquote>\n<p>Un altre punt clau per als pacients amb MC que incloguin la civada en la seva dieta, \u00e9s que <strong>optin sempre per la civada etiquetada mitjan\u00e7ant la Marca Espiga Barrada<\/strong> (MEB):<\/p>\n<p><em>Il\u00b7lustraci\u00f3 2.
